My Medications List
My HealtheVet presents a blended view of medications used by the My HealtheVet user: those prescribed by VA clinicians that are extracted from VA medical records, those entered in the VA medical records by a VA a staff member and those self-entered that are non-VA prescriptions as well as over-the-counter (OTC), herbals and supplements.
When this My Medications List is printed and provided to your health care provider, it can provide additional information for clinicians that might otherwise be unavailable, and which has the potential to alert clinicians to a possible life-threatening drug interaction.

Add a Medication - Click this link to display the My HealtheVet Meds, OTC, Herbals, and Supplements page titled: Add Self-Entered Medications + Supplements (Personal Health Journal).
Refill a Prescription - Click this link to display the My HealtheVet Rx Refill page titled: Refill Prescriptions.
Filter Results - The medications listed can be filtered by Source or Status of the prescription. Select one in each.

NOTE - The columns for Medication, Date, Status, Pharmacy, and Source can be sorted in ascending or descending order. To sort a column, click on a triangle ( or )
Medication - This column displays the Rx number, name, strength and instructions of the medication, or the name of the herbal, OTC, or supplement. Column also displays the Category and expiration date.
Date - This column displays the Fill date of the VA Medication, and for self-entered items, and the start date that was entered by the user, and for Medications documented by VA, the start date from the VA medical record.
Status - This column displays the status of the prescription. Select the icon on the column header to see the definition of the terms.

For VA Medications, this column will also display the Quantity and Refills remaining.
Pharmacy - This column displays either the VA Medical Center that originally dispensed the medication, the VA Medical Center that documented the medication or a self-entered pharmacy name.
Source - The source is displayed as either Self-Entered or VA.
I want to - This column displays a link to View details or Track Delivery (If available). View Details link will display the details page for the selected medication. Track Delivery link is shown in this column if the prescription was filled by the VA Mail Order Pharmacy and mailed in the last 15 days.
Number of Items Displayed - The summary page displays 10 rows. When there are more than 10 rows, you may choose from the drop-down menu to display 25, 50, or 100 rows on one page.
Self-Entered Information (SEI) Detail Page
NOTE - Only Name, Category and Start Date are required for self-entered medications and supplements.
Name - The name of the self-entered medication is displayed.
Patient Name - Full name of the user is displayed.
Date of Birth - Date of birth of the user is displayed.
Medication Start - A start date is displayed in this field.
Medication End - A stop date may be displayed in this field.
Status - A status of Active or Expired is displayed.
Category - The category, i.e., Herbal, OTC, Rx Medication, Supplement, or Other is displayed.
Strength - The strength of the item may be displayed in this field.
Dose - The number of units taken at a time (e.g., 2 tabs, 3 ounces) may be displayed in this field.
Frequency - The frequency of the dosage may be displayed in this field.
Pharmacy Name - The name of the pharmacy where the medication was dispensed may be displayed.
Prescription Number - When appropriate, a prescription number may be displayed in this field.
Pharmacy Phone Number - The phone number of the pharmacy that dispensed the item may be displayed.
Reason for Taking - A reason for taking this item (e.g., prescribed by doctor, for headaches) may be displayed.
Comments - Additional comments may appear in this field.

VA Medications Detail Page
Print - Click this link to display a Printer Friendly page.
Medication - Name of medication is displayed in the title of the page.
Patient Name - Full name of the user is displayed.
Date of Birth - Date of birth of the user is displayed.
RX # - Prescription number for the VA dispensed medication is displayed in this field.
Instructions - Instructions for the medication are displayed in this field.
Fill Date - For an Active prescription, the Fill Date is the date that the prescription was last refilled. Otherwise, the Fill Date indicates the next possible refill date. When a prescription has never been refilled, the Fill Date is the date that it was first issued.
Dispensed On - This is the date that the prescription is released for delivery via the Consolidated Mail Outpatient Pharmacy (CMOP).
Ordered On - This is the date that the prescription was originally issued.
Expiration Date - This is the date that the prescription is set to expire.
Status - This is the current status of prescription.

Category - This field displays “RX Medication”.
Source - This field displays “VA”.
Quantity - This is the number of prescriptions being refilled.
Refills Remaining - This indicates the number of refills remaining for the prescription.
Pharmacy - This is the name of the VA facility where the prescription was originally filled.

Non-VA Medication Detail Page
Note: Non-VA medication is a medication that came from someplace other than a VA pharmacy. This may be a prescription from either the VA or other providers that was filled outside the VA. Or, it may be an over the counter (OTC), herbal, dietary supplement or sample medication.

Medication – Name of medication is displayed in the title of the page.
Patient Name – Full name of the user is displayed.
Date of Birth – Date of birth of the user is displayed.

Documented Date – Date that the medication was added to the VA medical record.
Documented by – Person that added the medication to the VA medical record.
Medication Start - This is the date that the medication was started.
Status - This field displays “Active: Non-VA”.
Category – This field displays “Documented by VA”.
Source – This field displays “VA”.
Quantity - This is the number of prescriptions being refilled.
Documented at - This is the name of the VA facility where the medication was documented.
Comments – This is the comments added by the provider entering this in the VA medical record.

The Print page provides a printer-friendly format, designed to print on an 8-1/2 by 11 inch sheet of paper. The information on this page consists of the entries displayed on the previous screen. (To print different information, return to the previous screen, make your selection and then click the Print link to return to this page.) A border, indicated by the phrase "The printable information appears within this border", outlines the main content area. The only information that will print is inside the border although other information may be outside of the border.
